Optimus Healthcare is looking for an APRN or PA with Child & Adolescence experience to join our MDE team in Bridgeport, providing medical assessments, physicals, and recommend treatment. This grant funded position requires 16 hours per week scheduled for Tuesdays and Thursdays. Join our team-based model of care delivered in a kind and compassionate manner.
The Multidisciplinary Evaluation ( MDE) is a comprehensive, trauma informed screening that looks at the physical, dental, developmental, behavioral and emotional health of a child to:
Identify, assess and recommend treatment for any acute or chronic medical, educational, developmental, dental or behavioral health condition in a child placed outside of his or her home and identify any child who could benefit from a more comprehensive trauma-focused assessment.
Identify health condition that requires prompt medical attention such acute illnesses, chronic diseases requiring therapy (e.g. asthma, diabetes) signs of infections, nutritional problems developmental or mental health conditions and/or ongoing treatment needs.
EDUCATION: Graduate of an accredited school of nursing and successful completion of an approved Nurse Practioner program, or successful completion of Physician Assistant Program and licensed in Connecticut
EXPERIENCE: One-three years’ experience in the area of pediatrics .
LANGUAGE SKILLS: Bi-lingual skills in Spanish preferred
