Job description:
ALL APPLICANTS MUST HAVE 1 year Traveler and 1 year Home Care experience.
1 year Case Management Experience.
Ability to travel to cases as assigned.
Job Summary:
As a Home Health RN, responsibilities not limited to assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families.
Patient care is delivered within organizational philosophy, policy and standards of community health practice.
Also required to perform admission processes and case management responsibilities in accordance with MGB Home Care’s policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.
Home health nurses can have a nursing diploma, be a licensed practical nurse, have an associate of science in nursing, a bachelor of science in nursing, or higher. Some executive home health nursing positions such as those in administration or nursing management could require master's level education. Some organizations, like the American Nurses Credentialing Center offer certifications in home health care.
