POSITION: SLS Direct Support Professional
HOURS: Full-Time, Part-Time
PAY: $21–$26/hour

OVERVIEW
Optimum Care SLS II is seeking an experienced, patient, and compassionate Direct Support Professional to help us provide supported living services (SLS) to clients in Sonoma County.
We are a supported living services (SLS) vendor for the North Bay Regional Center serving adults with intellectual and developmental disabilities.
As an SLS Direct Support Professional, you will work closely with an individual in their home and community, offer assistance with activities of daily living, promote independence, and ensure individuals' continued physical and psychological safety and well-being.
RESPONSIBILITIES
· Daily living assistance, including assistance with personal care, meal preparation, shopping, household chores, and recreation.
· Supporting the individual in building meaningful relationships with community members and their circle of support.· Medication administration and other medical assistance, to the degree required by the individual.
· Ensuring a safe, clean living environment for the individual that reflects the individual's personal preferences.
· Providing person-centered support and advocating for the individual’s needs and rights in a variety of settings.
REQUIREMENTS
· Minimum 1 year of caregiving experience (Experience in behavioral health or nursing is highly preferred)
· Valid driver’s license and insured vehicle.
· Able to pass DOJ/FBI LiveScan background check and DMV records check.
· CPR and First Aid Certificate(s).
· Negative TB test result from within the past year, or willingness to obtain.
