Position Summary:

 

The Case Manager provides support for children and adults participating in the APSS program by connecting them with resources, helping them navigate the adoption process, and assisting with meeting or finding resources to meet their needs while in the program.

 

Essential Duties and Responsibilities Include:

 

  1. Conduct intake process for newly referred children or adults.
  2. Accesses community-based resources appropriate to the family’s needs.
  3. Interfaces with Department of Children and Family Services, Department of Mental Health, court systems, schools, health care providers and other community liaisons.
  4. Assists family with developing a needs/strength assessment.
  5. Provides support and advocacy which are family-centered, community and strength-based.
  6. Provide Adoption-Focused parenting education for adults pursuing adopting a child through the APSS program.
  7. Documents client’s strengths and needs; facilitates the development of measurable goals and objectives related to client strengths and needs.
  8. Creates a plan of activities to be accomplished with the client and documents S.M.A.R.T. goals.
  9. Facilitates a monthly parenting support group and collaborates with team members regarding its implementation and promotes participation from clients.
  10. Provides parenting education.
  11. Follows states laws and agency guidelines regarding child abuse reporting.
  12. Maintains weekly service provision requirements.
  13. Active participation in administrative and clinical supervision and meetings.
  14. Is receptive in receiving feedback from children and families, community and service providers.
  15. Ensures accuracy in work and that all deadline expectations are met.
  16. Maintains consistent and regular attendance.
  17. Follows up with supervisor in a timely manner regarding significant issues.
  18. Demonstrates ownership of work responsibilities by incorporating feedback, taking initiative, and interacting in a professional manner within assigned role and adhering to agency guidelines.
  19. Maintains confidentiality as required by PLYS, licensing, HIPAA, and other regulatory agencies.
  20. Follows and adheres to all field safety guidelines.
  21. Other duties as assigned.
Qualifications:
  1. Minimum of a bachelor's degree.
  2. Experience providing support services / direct client services in the community.
  3. Bilingual (English / Spanish) preferred.
  4. Experience working in programs focused on Adoption preferred.
  5. Excellent verbal and writing skills.
  6. Ability to verbally communicate well with others.
  7. Be able to work no more than 20 hours per week
  8. Valid California driver’s license; current automobile insurance; have and maintain a clean driving record acceptable to the organization’s insurance company
  9. Must be able to be pass a fingerprint background investigation.
  10. Must be able to pass a TB test, Physical Exam, and Drug Screening.
  11. All employees, regardless of position, serve as role models for children and adolescents who are served by our Agency.  Therefore, each employee must at all times be emotionally stable and able to function effectively with children, adolescents and adults who may have mental or behavioral health problems.  The staff must be able to demonstrate appropriate daily behavior, appropriate expression of emotions, as well as appropriate role modeling. 

 

In accordance with CA law, the expected pay range for this CA position is posted with this position. The actual compensation may be determined based on bona fide factors such as education, experience, and other factors permitted by law.

Hiring Range for this position: $22.50 - $22.50 / hour (Starting Pay Range)
