Optimal Health & Wellness, Northwest Indiana's premier functional and integrative medicine practice, is expanding our clinical team. We are seeking a passionate, skilled, and experienced Functional Medicine Nurse Practitioner to join us in caring for patients with complex chronic medical conditions using a holistic, root-cause approach.

We specialize in advanced functional medicine diagnostics, natural treatment options, IV nutrient therapy, ozone, hyperbaric oxygen, metabolic health, hormone optimization, and chronic illness support—including Lyme, mold toxicity, autoimmune conditions, and gut dysfunction. Position Highlights

• Flexible Schedule: 1–2 days per week to start, with opportunity to expand as the practice continues to grow.
• Location: In-person at our Chesterton, Indiana clinic or virtual visits. Fully remote is acceptable if licensed in Indiana.
• Contract Type: Independent contractor
• Compensation: Paid per visit commission opportunities on supplements, programs, and procedural referrals.

Responsibilities

• Provide high-quality functional medicine evaluations and follow-up visits (in-person or telehealth).
• Create individualized treatment plans using evidence-based integrative protocols.
• Manage and support patients with complex chronic medical conditions including:
• Autoimmunity
• Lyme & coinfections
• Mold/mycotoxin illness
• Hormonal imbalances
• Metabolic dysfunction
• GI disorders
• Interpret advanced functional lab testing (DUTCH, GI-MAP, Vibrant panels, OAT, mycotoxin testing, micronutrient testing, etc.).
• Recommend and prescribe supplements, lifestyle interventions, and adjunctive therapies.
• Collaborate with our wellness team, including IV nurses, HBOT techs, and lymphatic/ozone providers.
• Conduct charting, ordering, patient messaging, and follow-up care within EMR guidelines.
• Participate in ongoing education and case collaboration as part of a dynamic functional medicine environment.

Required Qualifications

• Nurse Practitioner (FNP, AGNP, or equivalent) with active Indiana license (or ability to obtain).
• Minimum 2 years of functional medicine experience or advanced training (IFM, A4M, Kalish, or equivalent).
• Proven experience treating complex chronic conditions using a root-cause, systems-based approach.
• Strong working knowledge of IV nutrient therapies, ozone therapy principles, and nutraceuticals.
• Comfortable practicing independently in a cash-based model.
• Excellent patient communication, education, and documentation skills.
• Entrepreneurial mindset and excitement to grow with a high-demand practice.

Preferred Qualifications

• Experience with chronic Lyme, mold exposure, MCAS, metabolic testing, BHRT, and detox protocols.
• Familiarity with IV ozone, HBOT treatment integration, and injectable peptides.
• Previous work in integrative, naturopathic, or concierge practice settings.
