COORDINATORS OF COMMUNITY SERVICES (CCS / Case Manager)
OPTIMAL CASE MANAGEMENT collaborates with the Developmental Disabilities Administration (DDA) of Maryland to coordinate the supports and services of Maryland residents with intellectual and developmental disabilities. All staff complete job duties following the Optimal Case Management program and policies, and DDA's mission, vision, and philosophy.
This role provides person-centered case management for individuals receiving DDA services, including developing plans of service, coordinating resources, and advocating for participant well-being. The position requires timely communication, accurate documentation, and adherence to all regulatory, ethical, and agency compliance standards. Work is primarily remote with some community travel, and involves collaborating with interdisciplinary teams, responding to crises, and supporting participants with professionalism and compassion.
Minimum requirements:
Bachelor's degree in a human service discipline AND two years of experience in human services, with a preference for case management experience
An understanding of developmental disabilities
The ability to travel throughout your assigned area, with a preference for those who live in the area
