Background Screening

What You'll Do
- Travel to patient homes and facilities within the assigned service area
- Assist patients with activities of daily living (bathing, dressing, grooming, toileting)
- Support safe transfer, ambulation, positioning, and comfort measures
- Assist with meals, hydration, and basic care-related homemaking
- Take and document vital signs as directed
- Observe and report changes in patient condition to the RN
- Follow individualized Hospice Care Plans while adapting to assignment changes and varying care environments
- Document care provided and communicate professionally with patients, families, facility personnel, and the care team
- Must hold current certification as a nurse aide and be in good standing on the state registry
- Minimum 1 year of experience as a hospice aide, home health aide, or certified nursing assistant preferred
- Valid driver's license and reliable transportation with valid insurance
- Willingness to travel frequently within the assigned service area
- Ability to read, write, and communicate effectively in English
- Ability to perform the physical demands of the role, including standing, bending, lifting, and use of transfer equipment
