Job Summary
Optimal Care Services Inc. is seeking an experienced Designated Coordinator / Program Coordinator to support our Minnesota 245D program. We are looking for a dependable, organized, and knowledgeable professional with at least 2 years of experience in the human services, disability services, mental health, vulnerable adult, HCBS, or 245D field.
The Designated Coordinator will help oversee service delivery, support direct care staff, track required client meetings and documentation, communicate with case managers and families, and ensure services are provided in a person-centered and compliant manner.
This position is ideal for someone who understands 245D services, can manage deadlines, is comfortable working with staff and clients, and has strong documentation and communication skills.
Key Responsibilities
- Coordinate and monitor services for individuals served under the 245D program.
- Help ensure services are delivered according to each person’s support plan, support plan addendum, goals, needs, and preferences.
- Track and assist with required client meetings, including 45-day meetings, semi-annual reviews, annual reviews, and other service-related timelines.
- Communicate with case managers, clients, families, staff, and leadership regarding service updates, concerns, and follow-up needs.
- Support Direct Support Professionals with client-specific instructions, documentation expectations, service goals, and support plan implementation.
- Observe and assist with evaluating staff service delivery and competency when needed.
- Review documentation for accuracy, completeness, and compliance.
- Assist with admissions, service changes, client onboarding, and program transitions.
- Help identify service delivery concerns, documentation gaps, staffing issues, and compliance risks.
- Assist with incident follow-up, emergency response coordination, and internal quality assurance.
- Maintain organized client records, tracking tools, and documentation logs.
- Attend required internal meetings and complete assigned weekly and monthly program tasks.
Required Qualifications
- Minimum of 2 years of experience in human services, disability services, mental health services, vulnerable adult services, residential services, HCBS, or 245D-related work.
- Knowledge of Minnesota 245D services and DHS licensing expectations preferred.
- Experience working with individuals with disabilities, mental health needs, behavioral support needs, or vulnerable adults.
- Strong documentation, organization, time management, and follow-through skills.
- Ability to communicate professionally with clients, families, case managers, staff, and leadership.
- Ability to support, coach, and give direction to direct support staff.
- Ability to use electronic documentation systems, Microsoft Office, Outlook, Excel, and internal tracking tools.
- Must be able to pass a DHS background study.
- Valid driver’s license, reliable transportation, and ability to travel to client or service locations as needed.
Preferred Qualifications
- Prior experience as a Designated Coordinator, Program Coordinator, Program Supervisor, Designated Manager, QP, Case Manager, 245D Coordinator, or similar role.
- Associate degree, bachelor’s degree, diploma, or training in human services, social work, psychology, education, healthcare, nursing, disability services, or a related field.
- Experience with 45-day meetings, semi-annual reviews, annual meetings, service agreement tracking, client goal tracking, audit preparation, and staff training.
- Knowledge of person-centered planning, positive supports, client rights, mandated reporting, vulnerable adult protection, and service documentation standards.
Benefits
- Pay range: $24.00 – $26.00 per hour
- Sick and Safe Time accrual in accordance with Minnesota requirements
- Paid training
- 12 hour PTO approved upon hired date, and then one hour per 80 hours worked to be accrued
- Mileage reimbursement for approved work-related travel
- One remote workday per week, scheduled on a Friday, subject to program needs and supervisor approval

Schedule
- Monday to Friday

- Primarily in-person position due to program, client, staff, and compliance needs
- One Friday per week may be worked remotely, subject to prior approval and business needs
