Position Overview: As a Licensed Practical Nurse (LPN) at Optimae Home Health Services, you will provide high-quality nursing care to our patients in their homes. You will be responsible for implementing care plans, monitoring patient health, and collaborating with the healthcare team to ensure optimal patient outcomes. This role offers flexibility, typically working Monday through Friday, 8:00 am to 4:30 pm, with minimal to no nights, weekends, or holidays.

Key Responsibilities:

• Implement and manage individualized care plans.
• Conduct patient assessments and monitor their condition.
• Administer medications and treatments as prescribed.
• Educate patients and their caregivers and support team about health conditions and care plans.
• Collaborate with RNs, physicians, and other healthcare professionals.
• Ensure compliance with all regulatory standards and company policies.
• Maintain accurate and thorough documentation.

Qualifications:

• Active LPN license in the state of Iowa.
• Reliable transportation and a valid driver’s license is required
• Strong communication and organizational skills.
• Ability to work both independently and collaboratively.
• A passion for providing home care and supporting mental/behavioral health needs.
