ABEM-certified physicians actively participating in ABEM MOC: No additional merit badges required.

Board Eligible physicians: ACLS, ATLS, and PALS required; AHA preferred. BLS required.

• Case logs covering the past 2 years

• CME documentation for the past 2 years

• Current-season flu vaccination

• TB documentation from the past 12 months or applicable chest X-ray

• Health attestation completed by PCP or peer during credentialing

• $100K/$300K malpractice coverage

• Louisiana Patients Compensation Fund (LPCF) enrollment required; locums physicians must maintain enrollment for a minimum of 90 days at a time

Cross-credentialing is required at four affiliated locations.
