DIRECT SUPPORT PROFESSIONAL-CROSS FUNCTIONAL

DEFINITION: Primary responsibility will be to support individuals by providing choices that enhance their quality of life. You will assist and support each individual with personal day-to-day living needs. Including: habilitation, building community supports, supervision, and health and safety.

RESPONSIBLE TO: Lead Supported Living Coordinator

Responsibilities of the DSP-Cross Functional:

  • Effectively support and assist every individual based on their support needs.
  • Consistently work with all staff members to meet the needs and aspirations of each individual.
  • Maintains appropriate professional boundaries.
  • Support individuals in self-care and provide care as necessary for those unable to care for their own needs.
  • Maintain functional working knowledge of individuals served.
  • Provides emergency and on-call services by ensuring adequate staffing needed to provide assistance to individuals.
  • Fill shifts as necessary.
  • Maintains/monitors the safety and the cleanliness of the individual’s home.
  • Monitors and/or assists with the self-administration of medications for individuals and maintains accurate documentation.
  • Contributes to individual assessments to enhance life skills of the individual based on abilities and strengths and give input on Plans of Care.
  • Responds to individual’s changing needs.
  • Communicates to the Supported Living Coordinator the individual’s need for additional supports.
  • Effectively implement calming techniques when responding to the emotional needs of individuals served.
  • Ensure individuals’ health and safety needs are always met.
  • Provide direct support and supervision to individuals per their plan of care.
  • Provide instructional support and assists in areas of health and safety, personal hygiene, meal planning and preparation, home management, finance, shopping, emergency procedures, problem solving, coping skills and communication.
  • Assist in providing opportunities for community integration and inclusion through thoughtful ideas and planning, and assistance as needed.
  • Supports/supervises individuals on community events/outings, medical appointments, and to/from work.
  • Provides support to Community First Choice (CFC) Nurse Supervisor by ensuring that all paperwork required in the CFC program is maintained in each individual’s file and available for review when needed.
  • Ensures the availability of CFC Service Delivery Records (SDR) for staff documentation, reviews all CFC data in the EVV system, comparing to staff documentation on the SDR and submits data for review for billing purposes. Provides CFC data to billing department.
  • Performs other duties as assigned.

General:

1. Maintain appropriate attendance.

3. Maintains required certifications.

4. Meet Annual Training requirements.

5. Attends all mandatory orientation and training activities, scheduled staff meetings and in-services/training as requested by Supervisor.

6. Establishes and maintains effective working relationships with individuals, co-workers, parents, agencies, and the public.

7. Maintain professional and appropriate boundaries.

8. Uses communication skills to participate effectively as a team member.

9. Uses equipment and supplies efficiently and informs Supervisor of shortages/needs.

10. Documents accurately work performed on Paycom and submit in a timely manner to Supervisor for approval.

11. Documents in Data Management Systems accurately and in a timely manner.

12. Safeguards and keeps confidential all Private and Personal Information regarding individuals with disabilities received in the course of their employment by the Corporation.

13. Practice safe work habits and follow all safety regulations and company procedures.

14. Maintain strict confidentiality.

Qualifications:

  • High School diploma or equivalent and prior experience working with individuals with disabilities preferred.
  • No record of Abuse/Neglect or Exploitation of others.
  • Must have or obtain a valid Montana driver's license, have an acceptable driving record, be insurable and present proof of private auto insurance in compliance with Montana State law within 1 month of hire.
  • Complete all required trainings within 30 days of hire.

Special Conditions:

  • Physical effort is required to perform the following: lift or physically assist individuals in daily activities; restrain individuals (following our procedures); carry supplies; perform household cleaning and move furniture as needed. Must be able to lift 50 lbs. Standing, walking and bending as required. Indoor and Outdoor activities.
  • Exposure to unpleasant sights, sounds or odors and physical aggression may be encountered.
  • Exposure to communicable diseases and other conditions.
  • Flexibility with schedule as needed.

    Must Have: Valid driver license, no history of neglect, abuse or exploitation
    Extensive background checks will be completed.
