We are seeking a Residential Program Supervisor II to support people with disabilities to successfully live in their homes and participate in their communities.
You will be responsible for:
- Participating in the individual planning process with each person served
- Collaborating with a person's care team and facilitating team meetings
- Assisting with meal planning, grocery/personal shopping, & cooking
- Scheduling and attending medical appointments & providing medication administration
- Supporting & developing independent living skills
- Supervising a staff of Direct Support Professionals
- Providing training to personnel on person served health and support needs so individuals can live as independently as possible

To be successful in this role, you should have:
- A desire to make a difference in the lives of people with disabilities
- A designated coordinator may have a baccalaureate degree in a field related to human services, education, or health and one year of full-time work experience providing direct care services to persons with disabilities or persons aged 65 and older or equivalent work experience providing care or education to vulnerable adults or children
