  • Implement the mission of Opportunity Living by coordinating each client’s Individual Program Plan (IPP) in a manner that promotes client participation and enriches quality of life.
  • Take an active role in supporting the values of Opportunity Living.
  • Communicate with all staff, families, and the public in a courteous and professional manner. Perform duties in a manner that will produce a positive role model for staff and clients.
  • Conduct all client activities in accordance with safety practices and emergency procedures.
  • Serve as advocate for client rights and ensure value by promoting people-first language, actions, and services.
  • Ensure Comprehensive Functional Assessments (CFAs) are completed and updated on assigned clients per Interpretive Guidelines, compile and assimilate data for evaluations.
  • Schedule and facilitate thirty (30) day annual, special, and discharge team meetings and interpret results for clients and families or guardians.
  • Ensure that assigned clients have appropriate and active Interdisciplinary Team (IDT) members to meet individual needs. Promote client participation and choice and work with a team approach.
  • Ensure that clients’ strengths, needs, and priorities established by the IDT members are coordinated into the program plan for each client.
  • Review all formal programs and procedures written by IDT members to ensure all necessary components are included, and best programming practices are used prior to implementation.
  • Coordinate programming between home, vocational, and support services. Ensure implementation of new and revised programs in all areas of service.
  • Prepare program reports as needed, including program meeting reports and monthly progress notes.
  • Review data weekly from residential, vocational, and support services to ensure programs are being run consistently to identify problems. Ensure timely modifications are completed as needed.
  • Ensure program implementation and documentation are being carried out for assigned clients.
  • Monitor programming in classrooms a minimum of two (2) hours per week.
  • Maintain clients’ records including consents, social histories, anecdotal records, central file contents, and other pertinent information.
  • Be familiar with the medication regimen and adverse side effects of medications for clients on psycho-tropic medications.
  • Ensure the use of training opportunities involving clients in active treatment.
  • Assist in providing training on new and revised programs on an ongoing basis.
  • Supervisors obtain client approval for programs and procedures recommended by the team, and ensure that the programs incorporate the requests of the client and/or parent/legal guardian. Obtain consents from parents/legal guardian and Human Rights Committee for any restrictive measures.
  • Respond to concerns voiced by clients, families or guardians in a timely manner.
  • Act as a liaison between Opportunity Living and family members, funding agencies and other agencies/consultants in regard to the IPP process.
  • Work with the Director of Community Living regarding the placement of clients at Opportunity Living.
  • Facilitate program training during regularly scheduled house meetings.
  • Actively participate in the survey process conducted by the Department of Inspections and Appeals (DIA) or other surveying agencies.
    • Provide, locate, or clarify information to the surveyor as requested.
    • Know and follow regulations and standards that are necessary to maintain licensure
    • Assist in the development of the plan of correction regarding deficiencies in appropriate areas
    • Utilize information to implement necessary changes to continually work on quality improvement of services provided.
  • Provide training to Opportunity Living staff on IPP process, the role of a QIDP, CFA,  behavioral management program, and other identified areas.
  • Participate in an established on-call system.
  • Coordinate schedule so there is maximum program training across all three (3) shifts in all houses.

o   In the event weekend training is required, the QIDP will coordinate with the supervisor to ensure completion of all required training in accordance with organizational expectations.

  • In conjunction with the Director of Community Living, other directors and support services, identify training needs of Opportunity Living in regards to the IPP process.
  • Provide regular communication, feedback, and information sharing with staff.
  • Assist in ensuring the effective and efficient operation of the organization by performing all duties as assigned by supervisor/manager.
  • Maintain professional competency by participating in continuing education, in-services, and personal growth experiences.
  • Protect the privacy of clients and staff by maintaining confidentiality of records and personal information and follow all policies and procedures under the Health Insurance Portability and Accountability Act (HIPAA).
