DETAILED SUMMARY:

Provides direct care coordination services to individuals who are referred by Community HUB. Uses Community Health Worker, Harm Reduction and Motivational Interviewing models of case management that is client driven, solution focused and facilitates change and movement toward greater stability and well-being. Specific duties include, but are not limited to, home and office visits, intakes and assessment, client education, the tracking of services, community outreach, and assisting in program development. Work is crisis oriented and requires expertise in triaging client needs to appropriately coordinate care.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

  • Provides care coordination services to individuals referred by the Community HUB.
  • Conducts intakes and performs assessments.
  • Maintains confidential case notes in care coordination database.
  • Develops and maintains cooperative working relationship with healthcare providers, community-based agencies and service providers.
  • Maintains knowledge of community resources and access points.
  • Serves as link between client and resources.
  • Provides reporting and tracking information for data management systems.
  • Ensures that all federal, state, agency and/or funder guidelines are met.
  • Assists in program development work.

BILINGUAL POSITIONS ONLY:

Specialize in providing services to clients and families who speak a language other than English as a primary language. May occasionally be asked to support with translating/interpreting a language other than English to assist potential or current clients.

OTHER RESPONSIBILITIES

  • Attends staff trainings and meetings as required.
  • Participates in Community Health Worker training offered by state and funder.
  • Other related duties as assigned.

EDUCATION, EXPERIENCE, TRAINING REQUIREMENTS:

  • Care coordination or case management experience in social service setting.
  • Experience coordinating with other professionals and social service agencies.
  • Experience in managing crisis situations and maintaining resiliency.
  • Effective verbal and written communication skills with a diverse population including the ability to manage difficult situations and maintain resilience.
  • Knowledge or experience in delivering crisis-oriented services, programs, and/or resources to historically marginalized and underserved populations.
  • Experience in community organizing, education or outreach.
  • If hiring for a bilingual position. Must be able to speak, read, and write with proficiency and fluency in the required language.
  • Valid Washington State driver’s license and five-year driving history within Opportunity Council’s Auto Insurance Standards.

SKILLS AND ABILITIES REQUIRED:

  • Verbal and written communication skills.
  • Computer skills, which include word processing, data entry, e-mail, etc.
  • Interpersonal skills and the ability to interact positively with clients from diverse populations.
  • The ability to create an innovative approach or new procedure or system as appropriate.
  • Ability to maintain accurate and timely case management records.

WORKING CONDITIONS:

Work is generally performed in a fast–paced office environment with frequent interruptions and occasional crisis situations. Frequent home visits off site. Frequently requires travel within the agency’s service areas for home visits, community resource networking, meetings, special workshops, and other job-related professional activities.
