Job Summary
Open Water Counseling & Recovery is seeking a compassionate and motivated full-time Psychiatric Nurse Practitioner (PMHNP) to join our growing team. This role provides psychiatric care to residents in nursing homes and assisted living communities, delivering meaningful, relationship-driven care directly where patients live.

Responsibilities:
- Conduct psychiatric evaluations and follow-up visits
- Diagnose and treat mental health conditions in geriatric populations
- Prescribe and manage psychotropic medications
- Collaborate with interdisciplinary teams (therapy, nursing, facility staff)
- Provide ongoing assessment and adjustment of treatment plans
- Participate in care coordination and communication with families when appropriate
- Maintain timely and accurate documentation

Qualifications:
- Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP)
- Active Ohio APRN license
- DEA license (or ability to obtain)
- Experience in geriatrics, long-term care, or behavioral health preferred
- Strong communication and team collaboration skills
- Passion for working with underserved populations
