Job Summary
Open Hearts Home Care Service LLC is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide one-on-one shift care to pediatric clients in their home setting. The LPN will deliver skilled nursing care tailored to the medical, developmental, and emotional needs of pediatric patients, working closely with families, physicians, and the interdisciplinary care team to ensure safe, high-quality care.
Key Responsibilities
- Provide direct skilled nursing care to pediatric clients per the physician's plan of care, including medication administration, tracheostomy care, ventilator management, G-tube/feeding care, suctioning, and other prescribed treatments as applicable to the client's condition.
- Monitor and document vital signs, symptoms, and changes in the child's condition; escalate concerns to supervising RN/Clinical Director and physician as needed.
- Administer medications accurately and in accordance with physician orders and agency policy.
- Maintain accurate, timely, and complete clinical documentation in compliance with agency, state, and Medicare/Medicaid EVV requirements.
- Support age-appropriate developmental activities and safe engagement during shift care.
- Communicate professionally and compassionately with parents/caregivers, ensuring they are informed and involved in the care process.
- Respond appropriately to emergencies, including CPR/first aid as needed, and follow the client's emergency care plan.
- Adhere to infection control, safety, and universal precaution standards.
- Coordinate with the interdisciplinary team (RN Case Manager, therapists, physicians) regarding changes in care needs.
- Complete required shift documentation, EVV clock-in/out, and incident reporting accurately and on time.
Qualifications
- Active, unrestricted LPN license in the state of Pennsylvania.
- Current CPR/BLS certification.
- Pediatric home care, NICU, PICU, or pediatric clinical experience strongly preferred.
- Experience with trach/vent care, G-tubes, or other pediatric skilled care needs a plus.
- Current negative TB test/PPD and up-to-date immunizations per agency requirements.
- Clear criminal background check and child abuse clearances (PA State Police, PA Child Abuse History Clearance, FBI clearance as required).
- Reliable transportation and ability to travel to client homes within the assigned service area.
- Strong communication, documentation, and time-management skills.
- Ability to work independently while following physician orders and agency protocols.
Availability
- Flexible scheduling: 4-hour, 8-hour, and 12-hour shifts available.
- Days, evenings, nights, and weekend availability needed to meet client care schedules.
Physical Requirements
- Ability to lift/transfer pediatric clients as needed (with appropriate technique/equipment).
- Ability to stand, bend, kneel, and move throughout a home care environment for the duration of a shift.
- Visual and auditory acuity sufficient to monitor client status and respond to alarms/equipment.
