Job Function:
The Continuum of Care Permanent Supportive Housing Program (CoC) is a federally funded initiative by the Department of Housing and Urban Development (HUD) aimed at providing permanent housing subsidies to individuals and families experiencing homelessness with documented disabilities. As the administrator of this program for Chester County, Open Hearth, Inc. oversees both the housing subsidy management and the supportive services necessary for long-term housing stability. The Housing Services Coordinator is responsible for coordinating housing operations, providing direct client services, engaging landlords, and ensuring program compliance and reporting.
Position/Reporting:
This is a full-time position reporting to the Housing Programs Manager.
Required Knowledge, Skills, and Abilities:
· Ability to manage complex situations with tact, judgment, and poise.
· Proven social work skills including direct service delivery, case documentation, resource coordination, and data reporting.
· Ability to work independently with accuracy and initiative.
· Proficiency in Microsoft 365 and related programs.
· Bachelor's degree in a relevant field and a minimum of two years of social work experience, including one year at a responsible coordination level. Or, 5 years or more of relevant work experience.
Additional Desirable Qualifications:
· BSW or higher in a relevant field or equivalent experience.
· Experience with Chester County's coordinated entry system, CCCIMS, HMIS, ACT, CTI, and the Partnership to End Homelessness.
· Familiarity with Fair Housing Law and HUD guidelines.
· Bilingual (English/Spanish).
Duties and Responsibilities:
Direct Service:
· Provide housing stability case management and resource coordination.
· Conduct at minimum quarterly outreach to clients and crisis intervention as needed.
· Collaborate with landlords and mediate tenant issues.
· Engage with potential clients, identified through the Coordinated Entry System, to assess their program eligibility and facilitate application and enrollment process.
· Perform HUD Habitability Standard inspections and annual recertifications.
· Identify and support participants transitioning to less intensive subsidy programs.
· Create monthly rent and utility payment log and adjust rent based on income changes.
· Update Fair Market Rents per HUD guidelines.
· Ensure that all units meet Rent Reasonableness determination.
· Maintain and update participant/landlord documents.
· Ensure compliance with Fair Housing laws and HUD requirements.
· Collect utility bills monthly in order to determine if stipends are being applied to the utility bill.
Landlord Engagement & Community Building:
· Implement landlord recruitment and retention strategies.
· Conduct outreach and education on program updates and Fair Housing Law.
· Build collaborative relationships with community service providers and participate in relevant meetings and initiatives.
General:
· Promote and fulfill the organization's mission.
· Foster a team-oriented work environment and maintain positive relationships.
· Represent the organization positively to stakeholders and the community.
· Perform additional duties as assigned to meet program goals.
