Our QIDP (Qualified Intellectual Disabilities Professional) position monitors and directs the development, implementation, monitoring, and delivery of all individual services to ensure the optimum level of independence possible in the least restrictive setting for our individuals with developmental and intellectual disabilities
Here at Open Door we work day in and day out to ensure our individuals are well taken care of and get to experience life to the fullest. Our QIDP role is vital to ensuring we accomplish just this.
Job Functions:
- The primary purpose of the QIDP is to provide coordination and support for the services received by assigned individuals.
- The QIDP is responsible for developing and overseeing the delivery of services across settings in order to enhance the well-being, quality of life, opportunities and achievement of personal outcomes for individuals served on their caseload.
- Develops, monitors and documents person centered Implementation strategies (IS).
- Reviews service plans monthly, documents service implementation, progress, and needs for modification
- Monitor, implement, and/or change any and all behavior programs
- Promotes and teaches self-advocacy and advocates for individual.
- Monitors the health, safety and well-being and satisfaction of each consumer.
- Assures consumer's participation in all decisions affecting his/her life.
- Serves as primary contact person for guardians and all service providers
- Participates in a rotating on call schedule
Requirements:
Bachelor's Degree in Human Services field is required, including, but not limited to, social work, sociology, special education, rehabilitation counseling or psychology
Must possess a valid Illinois Driver's License.
One year experience of working with ID/DD population preferred, however not required
Must pass a criminal background check.
Be at least 21 years old
Pass a Pre-Employment physical with ability to push/pull/lift 50lbs
