Community Health Programs Coordinator
We are seeking a Community Health Programs Coordinator to lead and support two key public health initiatives: an Overdose Prevention/MOUD (Medication for Opioid Use Disorder) program and a Cardiovascular Health Innovation program. This role oversees grant requirements, manages program activities, and supports community health workers in delivering high-quality, culturally responsive services.
The ideal candidate will have a strong commitment to harm-reduction principles, community engagement, and reducing stigma related to substance use, as well as a passion for improving cardiovascular health outcomes. This position works collaboratively with clinical, administrative, and community partners to ensure effective program implementation and sustainable impact.
Responsibilities
Program & Grant Coordination
- Oversee day-to-day activities of the Overdose Prevention/MOUD and Cardiovascular Health Innovation programs.
- Ensure all grant requirements are met, including timely and accurate data collection, reporting, and documentation.
- Track program budgets and monitor expenditures.
- Support program planning, implementation, and continuous quality improvement.
Community Engagement & Outreach
- Conduct outreach and community education to support harm reduction, cardiovascular health awareness, and health equity.
- Build and maintain partnerships with community-based organizations, healthcare providers, and public health agencies.
- Represent the programs at community events, meetings, and coalition activities.
Staff & Team Support
- Provide direction and support to Community Health Workers in completing program activities and meeting program goals.
- Support a collaborative, trauma-informed, and culturally responsive team environment.
Training, Education & Advocacy
- Promote harm reduction approaches and contribute to reducing stigma associated with substance use disorders
- Assist with the development of educational materials and community-facing resources.
- Participate in training efforts for staff, partners, and community members as needed.
Administrative & Clinical Collaboration
- Work closely with clinical and administrative staff to coordinate services and integrate program activities into broader organizational workflows.
- Maintain detailed records, client data, and program files in accordance with grant and organizational standards.
Required Skills & Qualifications
- Knowledge of harm reduction principles and commitment to reducing stigma around substance use.
- Understanding of cardiovascular health, chronic disease prevention, or related public health concepts.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse communities.
- Experience with grant reporting, data tracking, or program coordination.
- Ability to track budgets and organize program activities.
- Comfortable performing outreach and engaging with populations affected by substance use and health disparities.
- Ability to work collaboratively across clinical, administrative, and community settings.
Preferred Qualifications
- Master of Public Health (MPH) or related graduate degree strongly preferred.
- Two years of experience in grant management, outreach, harm reduction, public health programming, or program development.
- Experience supervising or supporting community health workers or similar positions.
- Experience working with individuals from diverse socio-economic, racial, ethnic, and cultural backgrounds.
