RN Float Nurse PRN

Open Arms Home Health Care – Des Moines, is seeking a Registered Nurse with home care experience to fill the position of PRN Float Nurse for three locations in the Des Moines and Ames area. The position would encompass RN Case Manager duties, as well as performing other visits such as admissions, change in conditions, annuals, etc. There are no on-call requirements with this position. There may be more hours available in the future.

RN Float Nurse Job Description:

We are looking for a thoughtful and compassionate person who enjoys working with the elderly outside a clinical setting. Home care offers the ability to make a significant difference in the life of the patient. The position requires good time management skills while providing each patient with the time and attention necessary to meet his or her needs.

The float nurse will work in collaboration with the agency administrator and clinical manager, providing oversight of the delivery of services provided to agency clients at a three locations. Responsibilities include:

• Assessments/OASIS evaluations, recertifications and charting
• Admission, change in condition, annual, etc. patient visits
• Generate plans of care and assign appropriate cares
• Performing evaluation tasks, including vital signs and medication review
• Supervise provision of personal cares by home health aide staff
• Coordinate with therapists, physicians, care coordinators and other health care professionals involved in the patient’s plan of care
• Educate patients and their families on proper home health strategies and procedures
• Administrative duties, including branch staff supervision, hiring, firing and evaluations

The ability to multi-task and problem solve is essential. The successful candidate will possess strong clinical, communication, team-building and interpersonal skills.

RN Float Nurse Qualifications include:

• Active Iowa RN License.
• Strong clinical and assessment skills.
• Prior home care experience – preferred – employer will train.
• Experience with OASIS and Medicare/Medicaid regulations for Home Care – preferred – employer will train.
• Prior experience in QA, Performance Improvement or Nurse Education preferred.
• Strong computer skills and experience with EMR software – employer will train on software.
• Excellent communication, organizational and mentoring skills.
• Supervisory experience in home care.
