About the Role:

The Nurse Practitioner will serve as a vital healthcare provider delivering comprehensive family medicine services across diverse patient populations, including pediatrics, geriatrics, and individuals experiencing pain or trauma. This role focuses on conducting thorough clinical assessments to diagnose and manage acute and chronic conditions while prioritizing patient safety and quality care. The Nurse Practitioner will collaborate closely with interdisciplinary teams to develop and implement individualized treatment plans that address both physical and psychosocial aspects of health. By integrating evidence-based pain management strategies and trauma-informed care, the practitioner will enhance patient outcomes and support long-term wellness. Ultimately, this position aims to improve access to high-quality healthcare services within the community, ensuring compassionate and effective care for all patients served.

Minimum Qualifications:

  • Current and valid Nurse Practitioner license in the United States.
  • Master’s or Doctoral degree in Nursing from an accredited program.
  • Certification as a Family Nurse Practitioner (FNP) or equivalent.
  • Demonstrated experience in clinical assessments and patient management across family medicine, including pediatrics and geriatrics.
  • Knowledge of pain management principles and trauma care.
  • Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.

Preferred Qualifications:

  • Experience working in multidisciplinary healthcare teams.
  • Additional certifications in Pain Management or Geriatric Nursing.
  • Familiarity with electronic health record (EHR) systems and telehealth platforms.
  • Experience in community health or outpatient clinical settings.
  • Strong background in patient safety initiatives and quality improvement projects.

Responsibilities:

  • Perform comprehensive clinical assessments and physical examinations for patients of all ages, including pediatric and geriatric populations.
  • Diagnose and manage acute and chronic illnesses, utilizing evidence-based protocols and clinical guidelines.
  • Develop and implement individualized treatment plans, including pain management strategies and trauma-informed care approaches.
  • Prescribe medications and order diagnostic tests as appropriate, while monitoring patient progress and adjusting treatments accordingly.
  • Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care and ensure adherence to patient safety standards.
  • Educate patients and their families on health maintenance, disease prevention, and management of chronic conditions.
  • Maintain accurate and detailed patient records in compliance with regulatory and organizational policies.

Skills:

The Nurse Practitioner will apply clinical assessment skills daily to evaluate patient health status and identify medical conditions accurately. Expertise in family medicine enables the practitioner to manage a broad spectrum of health issues across all age groups, ensuring holistic care. Pain management and trauma care skills are essential for developing effective treatment plans that alleviate suffering and promote recovery. Strong nursing skills support patient education, safety, and advocacy, fostering trust and adherence to care plans. Collaboration and communication skills are critical for working with healthcare teams and patients to deliver coordinated, patient-centered care.
