Job Summary:
As a PRN Hospice RN, you will provide high-quality, patient-centered care to individuals with serious illnesses. You will work collaboratively with our interdisciplinary team to develop and implement individualized care plans, manage pain and symptoms, and provide emotional and spiritual support to patients and their families. This is a per diem position, allowing for flexibility and autonomy in your scheduling.
Responsibilities:
- Assess patients' physical, emotional, and spiritual needs
- Provide direct patient care, including wound management, medication administration, and symptom management
- Collaborate with patients, families, and interdisciplinary team members to ensure coordinated care
- Educate patients and families on disease processes, treatment options, and self-care techniques
- Document patient care and communicate with team members, physicians, and other healthcare professionals
- See patients in varied settings: homes, Assisted Living, and Long Term Care facilities
- Graduate of an accredited school of nursing.
- Current Registered Nurse licensure in the state of New Mexico
- Experience as an RN commensurate with one (1) of the following:
- One (1) year acute care experience within the last two (2) years
- One (1) year home health care experience within the last two (2) years
- One (1) year community health or public health experience within the last two (2) years
- One (1) year hospice experience within the last two (2) years
- Current BCLS certification.
- Current TB testing and pre-employment drug screening and passes a criminal background check.
- Valid state driver’s license and reliable automobile.
- Proof of current automobile insurance.
