Key Responsibilities
- Provide personal care services including bathing, grooming, dressing, and toileting
- Assist patients with mobility, transfers, and safe positioning
- Support comfort measures and promote quality of life
- Observe and report changes in patient condition to the supervising nurse
- Provide emotional support and respectful presence to patients and families
- Maintain accurate documentation of care provided
- Follow hospice care plans and agency policies
- Travel to patient locations in Santa Fe and surrounding communities
- Current CNA certification in the State of New Mexico (required)
- Hospice or home health experience preferred (not required)
- Compassionate and professional bedside manner
- Ability to work independently with strong reliability and communication
- Valid driver’s license and reliable transportation
- Ability to perform physical duties of the role (lifting, standing, assisting with transfers)
