Responsibilities:
- Provide assessment, counseling, and support to patients and residents to help them achieve their goals and improve their overall well-being
- Develop and implement individualized care plans that address the physical, emotional, and social needs of patients and residents
- Work closely with the interdisciplinary team to ensure comprehensive care
- Provide education and support to families and caregivers to help them navigate the healthcare system and provide continued support after discharge
- Document patient and resident records accurately and in a timely manner
- Stay up-to-date with industry best practices and regulations to ensure the highest level of care
- Licensed Social Worker (LMSW or LCSW) in the state of Texas
- Bachelor's or Master's degree in Social Work (BSW or MSW)
- Strong communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize tasks accordingly
- Compassion and empathy for patients and residents
- Ability to work as part of an interdisciplinary team
