Position Summary
The Hospice RN provides skilled nursing care, symptom management, and compassionate support to hospice patients wherever they call home. This role partners closely with the interdisciplinary team to ensure comfort, dignity, and excellent end-of-life care.
Key Responsibilities
- Case Manage hospice patients along with needed visits in Santa FE and surrounding communities
- Participates in nurse on-call rotation for weeknight coverage, Monday through Thursday. On-Call is typically one night per week, but may vary depending on the number of nurses in the rotation. Paid on-call stipend and additional after-hours visit pay are provided.
- Conduct patient assessments and manage pain/symptom control
- Administer medications and treatments per physician plan of care
- Educate and support patients, caregivers, and families through the hospice journey
- Collaborate with the hospice interdisciplinary team (IDT) including social work, chaplain, aides, and medical director
- Complete timely and accurate documentation in the electronic medical record
- Serve as an advocate for comfort-focused, patient-centered care
- Active RN license in New Mexico (required)
- At least 1 year of nursing experience (hospice/home health strongly preferred)
- Strong assessment, communication, and independent decision-making skills
- Ability to work autonomously in the field with excellent time management
- Valid driver’s license and reliable vehicle
- CPR certification preferred
