Hospice Nurse Practitioner (NP) - Dallas, TX

Join our compassionate hospice team in Dallas, TX, as a Nurse Practitioner (NP) providing essential support and care to our patients and their families. We are seeking a skilled NP to conduct face-to-face visits and collaborate closely with our hospice Medical Director to ensure comprehensive patient care.

Key Responsibilities:

• Conduct face-to-face visits with hospice patients in their homes or facilities to assess and support their clinical needs
• Collaborate with the hospice Medical Director and other healthcare professionals to ensure optimal patient care and continuity
• Evaluate and manage patient symptoms, adjust care plans as needed, and provide emotional support to patients and families
• Document patient encounters accurately and timely to ensure regulatory compliance
• Serve as a resource for clinical questions and assist in educating families and caregivers about the end-of-life process and available resources
  
  
  

Qualifications:

• Education: Master’s degree or Doctorate in Nursing, with current NP licensure in Texas
• Experience: Minimum of 1 year of experience as an NP in a hospice or palliative care setting
• Certification: Current Advanced Practice Registered Nurse (APRN) certification; CPR certification
• Skills: Strong clinical assessment and communication skills, with an empathetic approach to patient care
• Transportation: Reliable transportation and valid auto liability insurance (mileage reimbursement provided)
