Key Responsibilities
- Develop and execute strategic marketing plans to grow Del Corazon Hospice census in Albuquerque
- Build and maintain referral relationships with:
- Hospitals and discharge planning teams
- Skilled nursing and long-term care facilities
- Physicians, specialists, and clinics
- Assisted living communities and senior networks
- Conduct regular in-person outreach, education, and follow-up with referral sources
- Serve as a hospice resource by providing education on eligibility, services, and care coordination
- Collaborate closely with intake and clinical leadership to ensure smooth referral transitions
- Attend community events, facility meetings, and networking opportunities
- Track outreach activity, referral trends, and performance outcomes
- Represent Del Corazon Hospice with professionalism, compassion, and urgency
- Prior experience in hospice marketing, home health marketing, or healthcare business development preferred
- Established referral relationships in the Albuquerque healthcare community strongly valued
- Knowledge of hospice services, Medicare guidelines, and admissions processes
- Strong communication, organizational, and interpersonal skills
- Self-driven with the ability to work independently in the field
- Valid driver’s license and reliable transportation required
