Key Responsibilities
- Provide personal care and emotional support to hospice patients in SNFs and private residences
- Assist with activities of daily living (bathing, grooming, toileting, transfers, feeding)
- Observe and report changes in patient condition to the clinical team
- Support families with education and comfort measures
- Maintain accurate documentation per hospice and regulatory guidelines
- Work collaboratively with nurses, social workers, chaplains, and the interdisciplinary team
- Maintain patient dignity, privacy, and comfort at all times
- Travel throughout the service area daily
- Current New Mexico CNA license (required)
- Hospice or home health experience preferred
- Valid driver’s license, reliable transportation, and auto insurance
- Ability to travel daily within service area
- Strong communication skills and compassionate bedside manner
- Ability to work independently and as part of a team
- Must pass background check and drug screening
