Under the General Supervision of the Owners/Directors/Agency Nurse, Onyx Health Care Coordinator will perform the following duties
· Maintain and increase general knowledge for DDSD related standards
· Maintain and increase overall participant/program knowledge relating to Onyx Supportive Living
· Assist in maintaining Contractor/Employee program compliance for all Onyx Supportive Living program/s
· Attend all meetings/training designated by Onyx owners/Director/Agency Nurse to include but not limited to:
- Participant IDT meetings
- Quality Assurance Meetings
-DDSD trainings/meetings
· Assist in management/notification of Quality Assurance related task(s)
· Attending Participants medical appointments as directed by Agency Nurse
· Assist in training employees/contractors/participants as directed by Agency Nurse or identified designee (Side by Side, Hygiene)
· Respond to Urgent Care, Emergency Room and Mental Health facilities with participants as directed
· Review and Maintain participant medical files in Therap, Quick Mar, and Onyx Cloud.
· Assist in maintaining/creating participant medical charts and ensure Onyx compliance
· Complete monthly participant medical updates and email to the Participant’s Interdisciplinary Team.
· Assist in new hire training for medication administration side-by-side ( Received Meds, Destroyed Meds, Double Checks)
· Create and maintain a job related checklist
· Ensure the appropriate information is entered and maintained in the DDSD mandated data base (Therap Services)
· Computer skills to assist in the creation and completion of job related documents
· Assist in the transportation of participants for appointments, trainings, ISP outcomes, etc.
· Assist in the management/improvement of Contractors/Employees in the Corrective Action Process
· Assist in the creation, review and implementation of Medication Administration Reports (MARS)
· Assist with creation/implementation of participant Medication Consents
· Participate in the “A Moment’s Notice” program as directed by Agency Nurse and/or Onyx Management
· Monthly Medication Error Reports for all participants
· Assist in implementation of medication administration/medical action plans
· Participant wound care as directed by Agency Nurse
· Participant injections as directed by Agency Nurse
· Participate in Medical On-Call
· Participate in program coverage as needed
· Other duties as specified by Onyx Owners/Director
