JOB SUMMARY: Effectively mentor individuals seeking drug and alcohol addiction treatment and/or prevention. Provide peer support and case management services.
STATUS: This position non-exempt, full time
Peer Support:
- Act to address distress or a crisis by using knowledge of local resources, treatment, services and support preferences of peers
- Educates peers about health, wellness, recovery and recovery supports
- Advocates for the needs and desires of peers in treatment team meetings, community services, living situations, and with family
- Provide outreach to recovering individuals/families
- Listens to peers with careful attention to the content and emotion being communicated
- Demonstrates genuine acceptance and respect as well as an understanding of peers’ experiences and feelings
- Relates their own recovery stories to inspire hope
- Recognizes when to share experiences and when to listen
- Supports peers to use decision-making strategies when choosing services and supports
- Strives to resolve conflicts in relationships with peers and others in their support work
- Maintains a positive reputation in peer/professional communities
- Seeks opportunities to increase knowledge and skills of peer support
- Work collaboratively with staff to increase recovering individual’s involvement
- Focus of actions should encourage recovering individual’s/family’s empowerment and ownership, concentrating on natural supports and independence.
- Encourage recovering individual’s/family’s to continue natural supports
Case Management: Working knowledge of community resources, support services and referral systems.
- Provides concrete assistance to help peers accomplish tasks and goals
- Tailors services and support to meet the complexities and unique needs of peers and their families including cultural and spiritual beliefs and practices.
- Assist and supports peers to set goals and to dream of future possibilities
- Recognizes signs of distress and threats to safety among peers and in their environments
- Uses active listening skills as well as respectful, person-centered, recovery-oriented language in written and verbal interactions with peers, family members, community members and others
- Works together with other colleagues to enhance the provision of services and supports
- Recognizes the limits of their knowledge and seeks assistance from others when needed
Both:
- Document ALL contacts with client, family and community partners
- Demonstrate accountability of being open to feedback and developing process and information pathways
- Deliver life skills and trainings as assigned
- Provides feedback and insight to clinical staff on progress and improvement areas
- Perform all work tasks in a proper and safe manner per established policies, procedures and guidelines
- Comply with all federal state and agency health and safety reporting requirements
Experience and Skills Requirements:
Basic computer literacy
Fluent in Spanish
Experience with level of care systems including types of services OnTrack provides.
Familiarity with Electronic Health Records systems a plus.
Sufficient recovery to provide peer support services to clients
Education/Licensure/Certification requirement:
PSS, CRM or THW – Current Peer Support Certification
