AB 1207 - Completion of Mandated Reporter Training for Child Care Workers 2. LIC 501 - Personnel Record 3. LIC 503 - Health Screening (includes TB test) 4. LIC 9052 - Employee Rights 5. LIC 9108 – Suspected Child Abuse Statement 6. Immunization Records (MMR/Tdap/Flu) 7. Knowledge of Title 22 8. Pest Management Certificate of Completion 9. Pediatric CPR and First Aid Certificate/Card
