Performs professional work providing social work services to clients in the Community Alternatives Program unit of Senior Services, and related work as apparent or assigned. Work is performed under the moderate supervision of the Social Work Supervisor.

- Performs comprehensive assessments to identify all aspects of the beneficiary.
- Develops person-centered service plans to determine all formal and informal services.
- Provides multidisciplinary treatment team meetings with all providers to monitor health and wellbeing, and continued appropriateness of services.
- Completes monitoring requirements to include: telephone, face to face and electronic exchanges.
- Facilitates referral and linkage activities with medical, behavioral, social and other human services agencies.
- Manages a caseload of disabled adults.
- Performs assigned functions as a Disaster Ready Team (DRT) member, if appointed or volunteer.
- Performs related work as required.
