Nurse needed for ADvantage Program, An Oklahoma Home and Community-Based Medicaid Waiver Program
Job Summary:
ONHL Home Care is actively seeking a skilled and compassionate Nurse to cover Muskogee, Wagoner, and Okmulgee counties in Oklahoma preferably with specialized experience in the ADvantage Program. This role requires a dedicated and adaptable nurse who can deliver quality in-home healthcare, ensuring that clients receive the support they need to maintain independence and quality of life. The ideal candidate will bring a depth of clinical knowledge, patient advocacy, and flexibility to meet the unique needs of our clients.
Qualifications:
Must be licensed in the State of Oklahoma Registered Nurse
Should be skillful in organization and in the principles of time management and have knowledge of management processes.
Must be able to contribute to the quality of care being rendered through constructive communication with caregivers and staff.
Key Responsibilities:
- Conduct comprehensive in-home assessments of patients' health and wellbeing, monitoring vital signs, physical conditions, and mental health status.
- Identify changes in patient condition promptly and adjust care plans accordingly to address emerging needs.
- Implement nursing interventions in line with ADvantage Program guidelines to support each client's health and functional abilities.
- Act as a patient advocate, ensuring that clients understand their healthcare options and receive appropriate, personalized care.
- Provide guidance on chronic condition management, including diabetes, hypertension, or respiratory issues, to improve patient outcomes.
- Collaborate with case managers, social workers, and other members of the patient's care team to develop, review, and update personalized care plans.
- Coordinate with the ADvantage Program team to ensure all care provided aligns with state program regulations and goals.
- Complete thorough documentation of all assessments, interventions, and communications within the electronic health record (EHR) system.
- Maintain accurate and timely progress notes, reports, and updates to support ongoing patient care and ADvantage Program compliance.
- Report significant changes in patient conditions or concerns to appropriate healthcare team members, ensuring prompt response to critical needs.
Additional Responsibilities:
- Stay up to date with ADvantage Program regulations and any changes to state policies affecting home care services.
- Participate in training sessions, team meetings, and other compliance-related activities to support program adherence.
