BH Referral Navigator

One full-time position (40 hrs/week, 1.0 FTE) | Monday - Friday, 8:00 am - 5:00 pm | Main Livestock Exchange Campus (LSX), 30th & L | 

Job Duties

Within OneWorld, the Behavioral Health Referral Navigator helps patients manage and improve their health.  They assist low-income and uninsured patients in accessing integrated primary and behavioral health education and services.  They conduct basic screening for depression, anxiety, family violence, chemical dependency, and marital/family conflicts.  The Behavioral Health Referral Navigator assists patients with external referrals by connecting them with community resources and setting the goal for every patient to establish a medical home.  They address barriers to care, such as financial barriers, in order to decrease the stigma of seeking behavioral health care and address the lack of trust in medical providers.

Outside of OneWorld, the Behavioral Health Referral Navigator serves as a liaison between the clinic, the patient, and the community providers to ensure that quality and timely care is received.  They address the health care needs of the community through early intervention and outreach efforts.  The Behavioral Health Referral Navigator maximizes community opportunities for OneWorld to collaborate with local providers and organizations.  

• Integrated Healthcare Team Member:  Collaborates with the integrated primary care/behavioral healthcare team to ensure that the patient understands their treatment plan and financial resources.  Reviews the health record and executes directives and referrals from clinic providers and staff.  Schedules referral appointments at the appropriate specialty clinics and facilities.

• Documentation:  Uses the database and spreadsheets to document and expedite the referral process. Maintains confidential electronic health record.  Updates the status of the treatment plan and ensures records are sent to and received from referring facilities.  Faxes notes to referring facilities.  Follows up weekly with patients, as needed, and documents in the database the results of the patient contacts, referrals, and outcomes.  Reports progress of referrals to the referring provider. 

• Patient Navigator:  Facilitates services and makes systems of care approachable for patients regarding their physical, behavioral, and/or LGBTQ health care needs. Supports linkages and communication between the clinic and the patient. Assists patients with completing financial applications, registration forms, and other necessary paperwork.  Supports and interprets for the patients while coordinating referrals. 

• Community Resources:  Stays abreast of available services within the organization and community.  These include, but are not limited to:  outpatient psychotherapy clinics, eating disorder clinics, food bank resources, community PFLAG meetings, etc.  Responsible for updating and maintaining the resource binder.  Refers and/or prescreens patients to appropriate services.  Collaborates with local providers and organizations through referrals, partnerships, and events such as health fairs.

• Assistant to Behavioral Health Business Manager:  Assists with verification of billing information for scheduled encounters, Region VI screening, and completion of eligibility applications.  Makes copies and handouts at the direction of the BH Manager and Director.  Assists in meeting preparation, management, and maintenance of patient materials across clinics.  Maintains Code Black documents. Assists in multiple projects and initiatives in order to contribute to timely data collection and reporting, according to established goals and requirements.

• High school diploma or equivalent required.

• Associate’s Degree in Human Services, Social Services, Counseling, or a related field preferred.

• Proficiency in English and Spanish required; speaking, reading, and writing.

• Proficiency in basic computer software programs, such as Microsoft Office, required.

• Demonstrated ability to work with patients who have emotional, mental, and/or behavioral challenges.

• Demonstrated ability to communicate effectively with internal and external customers, with a high emphasis on interpersonal skills and building relationships with patients in medical and behavioral settings.

• Demonstrated ability to work as a team player, analyze and solve problems, prioritize and organize, exhibit a high level of integrity and initiative, and to work with a sense of urgency.

• Must have a working knowledge of the Latino culture.
