Key Responsibilities:
A. Supervision and Management:
- Supervise, mentor, and evaluate a team of LPNs to ensure high-quality skilled nursing practices.
- Conduct regular team meetings and one-on-one sessions to provide guidance, support, and professional development.
- Maintain compliance with organizational policies and regulatory requirements in all case management activities.
- Provide ongoing training opportunities to enhance nursing staff skills and knowledge.
- Oversee the development and implementation of individualized care plans for patients, with final review by RN supervisors.
- Collaborate with healthcare providers, patients, and families to ensure coordinated, patient-centered care.
- Ensure timely and appropriate delivery of medical, rehabilitation, and social support services.
- Address patient care concerns and escalate unresolved issues through the nursing leadership chain.
- Support business growth and revenue goals for the Nursing Department through outreach and recruitment initiatives.
- Facilitate home visits for meet-and-greets, intake assessments, and recertification evaluations.
- Ensure case management practices comply with applicable laws, regulations, and accreditation standards.
- Partner with the Nursing Operations team to conduct audits and maintain quality assurance.
- Implement quality improvement initiatives to enhance case management effectiveness.
- Stay up-to-date with healthcare regulations and best practices relevant to nursing case management.
- Serve as the primary liaison between the nursing team and other departments within the organization.
- Build and maintain strong relationships with healthcare providers, community organizations, support coordinators, and external partners.
- Communicate effectively with patients, families, and healthcare providers to ensure understanding and adherence to care plans.
- Manage scheduling, payroll, and other administrative tasks related to the nursing team.
- Prepare reports on nursing staff performance, patient outcomes, and relevant metrics.
- Licensed Practical Nurse (LPN) with experience in home health preferred.
- Strong leadership and team management skills.
- Knowledge of case management practices, healthcare regulations, and quality assurance processes.
- Excellent communication, problem-solving, and organizational abilities.
- Ability to coordinate multiple priorities and work collaboratively across teams.
- Office-based with regular travel to patient homes for intake and recertification visits.
- Standard business hours with flexibility as required for patient care and nursing team oversight.
