Position Summary

The IDD Program Specialist supports individuals with Intellectual and Developmental Disabilities (IDD) by coordinating, monitoring, and evaluating the services they receive. The role ensures person-centered care, compliance with regulations, and advocacy for individuals’ rights and needs. The specialist collaborates with families, supports coordinators, and community organizations to deliver appropriate and timely services. A thorough understanding of Office of Developmental Programs (ODP) regulations is essential for ensuring service quality and regulatory compliance.

Key Responsibilities

1. Individual Care and Service Coordination

  • Conduct assessments to determine individual needs and develop person-centered care plans.
  • Coordinate and facilitate appropriate services, including personal care, nursing, and respite.
  • Maintain collaboration with individuals, families, and supports coordinators.
  • Conduct outreach to community partners and organizations to promote available services.
  • Generate referrals and identify new opportunities to expand service reach.
2. Referrals, Staffing Coordination, and Onboarding

  • Work with regional managers and team leads to identify staffing needs for current and new referrals.
  • Oversee the referral process to ensure timely and accurate staffing alignments.
  • Review resumes, conduct preliminary interviews, and assess candidate qualifications.
  • Coordinate interviews and onboarding processes, ensuring completion of background checks and certifications.
  • Identify training needs and organize sessions to prepare new hires.
3. Service Coordination and Case Management

  • Implement and monitor Individual Service Plans (ISPs) collaboratively with families and coordinators.
  • Track service delivery to ensure compliance with ISPs and address evolving needs.
  • Continuously reassess care plans based on individual progress or changing conditions.
4. Compliance and Documentation

  • Monitor Direct Support Professionals (DSPs) to ensure service notes and records align with care goals.
  • Verify timely approval of service notes and ensure regulatory documentation is complete.
  • Maintain compliance with all state and federal laws, including Medicaid and ODP guidelines.
  • Ensure that participant and DSP profiles meet background check, certification, and documentation standards.
  • Report and document incidents according to regulatory requirements.
5. Advocacy and Individual Support

  • Advocate for individuals’ rights, dignity, and inclusion in their communities.
  • Encourage independence in daily living, employment, and social participation.
6. Training and Staff Supervision

  • Provide ongoing training and guidance to DSPs, families, and staff on best practices in care delivery.
  • Supervise DSPs to ensure adherence to service plans, documentation standards, and safety protocols.
Qualifications

Education:
  • Bachelor’s degree in Social Work, Psychology, Special Education, Human Services, or a related field preferred
Experience:
  • Minimum of 2 years of experience working with individuals with IDD in case management or service coordination
  • Familiarity with ODP waiver programs, Medicaid, and IDD-related regulations
  • Experience in person-centered planning and community integration highly preferred
Clearance Requirements:
  • Criminal Background Check
  • Motor Vehicle Record Check
  • Child Abuse Clearance
  • Authorization to work in the United States

  • Professionalism and empathy in sensitive situations
  • Strong case management and documentation skills
Reports to: IDD Team Lead

Work Environment:
  • Office-based role, Monday–Friday, 8:30 AM – 5:00 PM
  • Occasional travel to individuals’ homes, community settings, or healthcare facilities may be required
