Position Summary

The Licensed Practical Nurse (LPN) provides professional nursing care to individuals with Intellectual and Developmental Disabilities (IDD) in a home or community setting. Working under the supervision of a Registered Nurse (RN) or other healthcare provider, the LPN implements individualized care plans, administers medications, performs clinical procedures, and supports individuals and families in maintaining health, safety, and comfort. Familiarity with Individual Support Plans (ISPs) in Pennsylvania is required.

Key Responsibilities

  1. Individual Care and Support

Administer prescribed medications accurately and monitor for side effects.

Assist with personal care tasks such as bathing, dressing, grooming, and toileting when needed.

Monitor and record vital signs (blood pressure, pulse, temperature, respiratory rate).

Provide wound care, feeding tube management, catheter care, and other basic nursing procedures.

Support management of chronic conditions (e.g., diabetes, asthma, epilepsy) common among individuals with IDD.

Ensure comfort, dignity, and safety of individuals at all times.

  1. Care Plan Implementation

Follow individualized care plans developed by the RN Case Manager.

Report any health or behavioral changes promptly to supervising staff.

Contribute to the review and modification of care plans in collaboration with the interdisciplinary team.

  1. Health Monitoring

Observe and document changes in individuals' physical, emotional, and behavioral health.

Conduct regular health assessments and adapt care based on observed needs.

Recognize and respond appropriately to medical emergencies such as seizures, infections, or respiratory distress.

  1. Individual and Family Education

Educate families and caregivers on medication management, daily care, and health maintenance.

Provide guidance on promoting independence and quality of life for individuals with IDD.

Offer information about preventive care and the importance of regular health evaluations.

  1. Collaboration and Communication

Work closely with the RN Case Manager, physicians, and therapists to coordinate care.

Communicate regularly with families, caregivers, and interdisciplinary team members.

Participate in team meetings and case reviews to provide clinical input and updates.

  1. Documentation and Reporting

Maintain accurate and timely nursing records in compliance with healthcare regulations.

Document all assessments, interventions, medications, and incidents.

Ensure adherence to state, federal, and organizational documentation standards.

  1. Safety and Infection Control

Follow established safety and infection control protocols.

Practice proper hygiene and use personal protective equipment (PPE) appropriately.

Report safety concerns, hazards, or incidents to the RN Case Manager promptly.

Qualifications

Education:

Completion of a practical nursing program from an accredited school of nursing.

Licensure:

Current and valid LPN license in the state of employment.

Experience:

Minimum of 1 year of nursing experience required.

At least 1 year of experience working with individuals with IDD or in home health/long-term care preferred.

Reports to:

RN Case Manager

Competency Requirements

Strong clinical skills in medication administration, wound care, and health assessment.

Effective communication and interpersonal abilities.

Ability to monitor, document, and report changes in individuals' health conditions.

Knowledge of IDD-related health needs and support strategies.

Familiarity with home-care medical equipment (feeding tubes, catheters, oxygen therapy).

Work Environment

Positions available in in-home and community-based care settings.

Flexibility in working hours, including evenings, weekends, and overnight shifts, may be required.

Physical Requirements

Ability to assist with physical tasks, including lifting, transferring, and supporting individuals with mobility challenges.
