What You’ll Do
- Provide comprehensive, evidence-based primary care
- Perform Annual Health Assessments (AHA) and Chronic Care Management (CCM)
- Focus on prevention, education, and long-term health outcomes
- Collaborate with interdisciplinary teams to coordinate care
- Utilize EHR systems to improve workflows and patient outcomes
What You Bring
- Board Certified Physician with active California license
- Completed residency in relevant specialty
- 1–3 years of clinical experience (preferred)
- Strong communication and team collaboration skills
- Experience with EHR systems (Epic preferred)
