Description

The Veteran Case Manager provides comprehensive case management and supportive services to Veterans experiencing homelessness who are enrolled in One80 Place’s Grant & Per Diem (GPD) transitional housing program. The primary goal is to assist enrolled Veterans to create a housing plan to locate permanent housing as quickly as possible and ensure Veterans have a way to maintain housing with targeted referrals to services, resulting in greater self-determination. Functions performed include housing location, housing stability planning, connection to community resources, and housing problem solving. 


This position is funded in whole by One80 Place’s Grant & Per Diem grant.


One80 Place ends and prevents homelessness throughout South Carolina with offices located in Charleston and Columbia. This position will be based in Charleston.


STATUS: Full-time, Regular / Exempt

Requirements

SPECIFIC REQUIREMENTS:

  1. Bachelor’s degree in social work, psychology, human services, or related field preferred (or equivalent experience). 
  2. 2 years' experience working with Veterans, particularly those impacted by homelessness.
  3. Strong communication, de-escalation, and relationship-building skills. 
  4. Ability to work independently, manage time well, and adapt to crisis situations. 
  5. Excellent computer skills including basic office programs and the ability to learn and utilize the HMIS database. 
  6. Familiarity with housing programs, local resources, and HMIS is a plus. 
  7. Valid driver's license and reliable transportation. 
  8. Ability to travel throughout the 7-county service area of the Low Country Continuum of Care in a personal vehicle or company-provided vehicle. 

RESPONSIBILITIES

1. Veterans Coordinated Entry Responsibilities

· Participate in bi-weekly Veterans By-Name List meetings and case conferencing sessions.

· Review and discuss Veterans identified through outreach to assess needs and ensure accurate tracking on the By-Name List.

· Make referrals to appropriate programs in alignment with established coordinated entry guidelines.


2. Case Management Responsibilities 

· Meet weekly with enrolled Veterans to assess needs and monitor progress.

· Conduct comprehensive assessments to support Veterans in improving overall well-being and achieving housing stability.

· Facilitate access to behavioral health and medical care as part of the case management process.

· Ensure all Veterans receive information about available medical, mental health, and substance use services through the local VA and community providers.

· Assist Veterans with enrollment in H-PACT to connect them with comprehensive primary care and support services.

· Assist Veterans with medically complex needs navigate an appropriate housing plan.


3. Housing Case Management Responsibilities

· Assist participants in completing the Housing Barriers Assessment and Housing Preference Worksheet and develop housing plans tailored to each Veteran’s goals and housing needs.

· Connect participants to supportive services and mainstream benefit programs to promote stability and self-sufficiency.

· Refer Veterans to available permanent housing programs, when appropriate, to help them secure permanent housing.

· Initiate the housing location process.


4. Recovery & Relapse Support Responsibilities

· Intentionally engage Veterans to support recovery and address relapse risks, ensuring consistent connection throughout their participation in Bridge Housing.

· Recognize and respond to the impact of recovery and relapse on housing stability, adjusting service strategies as needed.

· Incorporate recovery-oriented approaches into housing plans to minimize the effects of substance use.

· Collaborate with Shelter Assistants and supportive service staff to keep Veterans engaged in the program and maintain housing upon program exit.


5. Documentation & Compliance

· Maintain accurate, timely documentation in HMIS in compliance with agency and funding requirements.

· Maintain compliance with outlined program requirements. 


6. Program & Team Collaboration

· Participate in team meetings, case conferences, and staff meetings.

· Partner with community providers and VA homeless programs.

· Contribute to program development, evaluation, and continuous quality improvement.

· Attend relevant training on topics affecting homeless Veterans.

· Ability to foster a “warm hand off” with case managers once the Veteran exits GPD.


PHYSICAL, ENVIRONMENTAL AND SENSORY DEMANDS:

  1. Requires sound mental reasoning, sound judgment, and the ability to respond calmly and effectively in a crisis.   
  2. Requires the ability to relate effectively to individuals experiencing homelessness.        
  3. Requires corrective vision and hearing to normal range. 
  4. Ability to move between service locations; ability to lift 25 lbs.   
  5. Possible exposure to communicable diseases and vicarious trauma. 

Salary : $55,000
