Position: Registered Nurse
Location: Hermiston, OR 97838
Duration: ASAP - 07/12/26 (possible extension)
Pay rate: $51/hr.
Setting: Home Health
Schedule:
40 hours weekly
Monday to Friday, 8 AM to 4:30 PM, with a rotating weekend schedule.
When scheduled for a weekend, the clinician will have Thursday off prior and Friday off the following week.
Overview: Join a compassionate team dedicated to delivering high-quality care where it matters most - in patients’ homes. As a Home Health RN, you will provide personalized nursing care, helping patients recover from illness, manage chronic conditions, and maintain independence in the comfort of their own environment
Key Responsibilities
Provide skilled nursing care to patients in their homes, including private residences, assisted living, and adult foster homes
Perform comprehensive patient assessments and develop individualized care plans
Monitor patient progress and adjust care plans to support optimal outcomes
Educate patients and families on disease management, medications, and recovery
Collaborate with physicians and interdisciplinary teams to coordinate care
Maintain accurate, timely documentation of assessments, treatments, and outcomes
Advocate for patients and support their safety, comfort, and well-being
Qualifications
At least 1-year of recent experience in acute care, critical care, emergency department, Community Health, Home health, or a similar setting is required.
Active RN License in Oregon
Active Basic Life Support (BLS) or be willing to obtain it post-offer
Must have a clean background
