- No DUIs in the Last 10 Years (5 Years if Non-CDL Holder)
- No More Than 1 DOT Preventable Loss in Last 2 Years
- No More Than 3 Preventable Accidents in Last 2 Years
- No Major Preventable Accidents in Last 5 Years
- No Serious Violations in Last Year
- Maximum 2 Moving Violations in Last 2 Years
- Must Pass DOT Physical & Drug Screen
- No Drug-Related Convictions in Last 5 Years
- No Failed or Refused Drug Tests in Last 10 Years
