Job Summary
Under the assigned CCS supervisor, the Care Coordination Support Worker provides community-based services to identified youth that are centered on Systems of Care and Wraparound services. CSSI plays a critical role in the community behavioral health service delivery system with the position ensuring that services are family-focused, youth-guided, individualized, strengths-focused and trauma-informed.
Essential Duties and Responsibilities
- Manages the CSSO Care Coordinator team
- Manage the referral intake for program daily
- Maintains staff to supervisor ratio of 8:1 (never to exceed 10 Care Coordinators)
- Provides oversight of ( Care Coordination Services Wrap Around) and CCSI( Care
Coordination Services Intensive) - Provides oversight of FSP and SFSP services and staff per program guidelines
- Position only supports Pathway programs
- Manages case assignments and intakes in order to ensure an equal distribution of
cases and appropriate staff-to-client ratios as outlined by the funding source - Oversees case responsibilities and provides guidance in case planning
- Provides weekly supervision to staff with a focus on Wrap, system of care approach and
trauma informed - Reviews all written documents created by staff, such as assessments, treatment planning,
notes, and other program documentation as required - Reviews and updates funder policies and procedures according to state requirements,
training staff on their implementation - Reviews, ensures consistency and daily filing of case documentation
- Ensures effective management of significant events and crises by staff
- Completes required outcome reports
- Participates in required meetings and trainings with funder
- Provides staff with training, education, ongoing supervision, support, and professional
development - Monitors program expenditures to ensure compliance with budget
- Ensures adherence to funder contract
- Ensures billing accuracy and timeliness; completes billing procedures for private
contracts when necessary - Handles client, staff, caretaker, and/or collateral complaints and grievances
- Conducts regular unit/team meetings
- Participates in collateral and community groups to promote agency programs and
presence within the community in order to create community linkages and opportunities - Participates in public speaking engagements to promote agency programs and services
- Participates in agency management meetings and work groups
- Works with the CQI team to ensure quality of services, the achievement of program
outcomes and to create documentation to enhance programming and to meet funder
requirements - Provides direct service to children and families in the absence of assigned staff
- Ensures adequate client contact is maintained in accordance with program
guidelines and that the minimum required contact hours are achieved each month - Ensures that all staff are monitoring child and client safety and are appropriately
responsive to any situation posing a risk to, or by, a client - Completes and submits performance appraisals in a timely manner Care Coordination Supervisor
