Duties
- Conduct comprehensive assessments of clients' needs, strengths, and challenges.
- Develop and implement individualized care plans in collaboration with clients and their families.
- Provide crisis intervention and management to clients experiencing acute situations.
- Facilitate access to community resources, including healthcare and mental health services, housing assistance and
- Collaborate with multidisciplinary teams to coordinate care for clients across various settings.
- Maintain accurate documentation of client progress and interventions in compliance with regulatory standards.
- Advocate for clients' rights and needs within the healthcare system.
Requirements
- Bachelor's degree in Social Work, Psychology, or a related field.
- Proven experience in behavioral health, crisis intervention, or related fields.
- Familiarity with motivational interviewing techniques and their application in client interactions.
- Experience in childcare or child welfare services is highly desirable.
- Knowledge of Medicare regulations and community resources available for clients.
- Excellent communication skills, both verbal and written, with the ability to build rapport with diverse populations.
