Type:Â Full-Time (40 hrs/week) - Monday through Friday 8am-5pm
Salary: $18.91-$19.84/hr DOE

Key Responsibilities:
- Coordinate Care:Â Facilitate warm hand-offs and referrals between patients, providers, and community services.
- Screen & Support:Â Conduct SDOH screenings (PRAPARE) and help patients set goals to overcome barriers.
- Educate & Advocate:Â Assist with applications for coverage, benefits, and other essential services.
- Document Thoroughly:Â Maintain detailed electronic patient records, tracking outcomes and service plans.
- Engage the Community:Â Participate in outreach events and consortiums to promote One Health services.
- Navigate Resources:Â Stay current on eligibility rules, insurance options, and enrollment procedures.
What We’re Looking For:
- Strong computer and documentation skills
- Ability to manage multiple priorities with remote supervision
- Professionalism, empathy, and clear boundaries
- Community involvement or connection to the population served
- High School Diploma or GED
- Valid Driver’s License
Certifications (upon hire or within first year):
- BLS Certificate
- Community Health Worker Certificate
- Certified Application Counselor (CAC)
