Registered Nurse (RN)- Care Team Case Manager

Join us at One Community Health (OCH) and make a difference in your local community! As a Registered Nurse (RN) at OCH you will be part of team-based care approach using the Patient Centered Primary Care Home (PCPCH) model.

This is a fantastic opportunity to build relationships and see the impact you can make in your community. The RN Care Team Case Manage triages, coordinates care, and mentors' staff to support quality care and continuous quality improvement initiatives.

This opportunity is a full-time position at 0.75 FTE (30 hours per week) at our clinic in The Dalles, Oregon. The preferred schedule will consist of three 10-hour shifts, ideally on Monday, Tuesday, and Friday.

Minimum Qualifications

• Graduate of an accredited nursing program
• Registered Nurse with current Oregon and Washington license
• Current BLS
  
  

Preferred Qualifications

• Previous FQHC of look-alike experience
• Bilingual English/Spanish
• Epic EMR (Electronic Medical Record)
• Experience as a supervisor or group leader in health care or other business.

Compensation

• Hourly/Non-Exempt: $40.00 per hour
• Bilingual Differential: Additionally, we offer $0.75 per hour as compensation for bilingual proficiency in both English and Spanish.
