Home Health RN Case Manager

We are seeking a Full-Time Home Health RN Case Manager who is confident in clinical assessment, care coordination, OASIS documentation, and developing individualized plans of care.

Position Responsibilities

As a Home Health RN Case Manager, you will:

· Provide skilled nursing care to patients in their homes

· Complete comprehensive assessments and ongoing clinical evaluations

· Develop, implement, evaluate, and update individualized Plans of Care

· Complete accurate and timely OASIS assessments and required documentation

· Identify changes in patient condition and implement appropriate interventions

· Collaborate with physicians, patients, families, caregivers, and other members of the healthcare team

· Provide patient and caregiver education related to disease management, medications, safety, and self-care

· Monitor patient progress toward goals and revise the plan of care as clinically indicated

· Participate in recertification, resumption of care, follow-up, and discharge planning

· Help prevent avoidable hospitalizations through proactive assessment, education, and coordination of care

· Maintain compliance with Medicare, agency, and applicable regulatory requirements

We are looking for an RN who is:

· Clinically confident and highly organized

· Detail-oriented with strong OASIS and documentation skills

· Able to work independently while functioning as part of a collaborative team

· Skilled in patient assessment, care planning, and clinical decision-making

· Compassionate, dependable, and committed to providing high-quality patient care

· Comfortable coordinating care with patients, families, physicians, and interdisciplinary team members

Qualifications

· Graduate of an accredited school of professional nursing

· Current, unrestricted RN license in the state of Texas

· Current driver's license and reliable transportation

· Current auto insurance
