Role Overview:
We are looking for a compassionate and detail-oriented Registered Nurse to join our Adult Foster Care (AFC) program as an Assessment Nurse. This role involves performing in-home evaluations for individuals applying to the AFC program, assessing their needs, and determining eligibility for services. This position is ideal for nurses with experience in home health, geriatrics, or community-based care who are committed to enhancing the quality of life for program participants.
Key Responsibilities:
- Conduct thorough in-home assessments for new AFC program applicants.
- Evaluate medical, functional, and psychosocial needs to determine eligibility.
- Collaborate with families, caregivers, and healthcare providers to develop individualized care plans.
- Educate participants and their families about AFC program services, guidelines, and expectations.
- Ensure compliance with state regulations and AFC program standards.
- Maintain accurate, detailed documentation of assessments, care plans, and follow-up activities in a timely manner.
