Pay Range: $22– $24 per visit
Job Summary:
We are seeking a dedicated and empathetic Case Manager to join our team in Lowell, MA. The ideal candidate will be passionate about supporting individuals in achieving stability, independence, and improved quality of life. This role involves conducting client assessments, developing care plans, coordinating services, and offering guidance to help clients reach their personal and financial goals.
Primary Responsibilities:
- Perform intake and psychosocial assessments to evaluate clients’ needs, strengths, and challenges.
- Create and manage individualized care plans, focusing on both immediate needs and long-term goals.
- Provide consistent support, counseling, and intervention to assist clients in overcoming barriers.
- Use motivational interviewing techniques to engage clients and promote self-sufficiency.
- Connect clients with appropriate community resources and social services.
- Offer coaching, mentorship, and life skills training to help clients build confidence and independence.
- Address crises with professionalism and employ effective de-escalation methods when necessary.
- Maintain thorough documentation and progress reports, revising care plans as client needs evolve.
