We are seeking a dedicated and compassionate Nurse Practitioner or Physician Assistant to join our dynamic healthcare team. Experience in family medicine or primary care is preferred. In this vital role, you will provide comprehensive patient care across a variety of settings, including outpatient clinics, inpatient units, and specialized care environments. Responsibilities
- Conduct thorough physical examinations, patient assessments, and diagnostic evaluations to determine health status and formulate treatment plans.
- Utilize EMR systems like Practice Fusion, or similar to document medical records accurately while ensuring compliance with HIPAA regulations.
- Collaborate with physicians and specialists in fields such as gastroenterology, infectious disease care, cardiology (including cardiac catheterization), ophthalmology, and occupational health to optimize patient outcomes.
- Provide patient education on health coaching topics including nutrition, family planning, post-acute discharge planning, and working with people with disabilities or developmental disabilities.
Experience
- Demonstrated competence in handling complex cases involving acute care, chronic care management.
- Familiarity with ICD-10/ICD-9 coding systems for accurate billing and utilization review processes.
- Experience working with telehealth platforms to deliver remote patient consultations effectively.
- Knowledge of occupational health protocols including DME (durable medical equipment), workers' compensation law, infection control standards, and utilization management practices.
- Ability to work across diverse group of patients in the clinic including adult, pediatrics, geriatrics, chronic conditions, acute emergency/walk in urgent care.
