Position Summary
We are seeking a licensed and patient-centered Physical Therapist (PT) to join our rehabilitation team with various locations in New Haven County. The Physical Therapist will be responsible for evaluating, diagnosing, and treating patients with musculoskeletal, neurological, cardiopulmonary, and functional impairments to improve mobility, reduce pain, and restore functional independence.
The ideal candidate demonstrates clinical excellence, evidence-based practice, strong communication skills, and a collaborative approach to interdisciplinary care.
Key Responsibilities
- Conduct comprehensive physical therapy evaluations and develop individualized treatment plans.
- Perform diagnostic assessments of mobility, strength, balance, gait, posture, and functional limitations.
- Implement evidence-based therapeutic interventions including:
- Manual therapy
- Therapeutic exercise
- Neuromuscular re-education
- Gait and balance training
- Modalities as appropriate
- Reevaluates patient progress and modify treatment plans as needed.
- Maintain accurate, timely, and compliant documentation in accordance with:
- Connecticut practice regulations
- Medicare/Medicaid guidelines
- Commercial payer standards
- Educate patients and caregivers on home exercise programs and injury prevention.
- Collaborate with physicians, occupational therapists, speech therapists, nurses, and case managers.
- Participate in discharge planning and transitional care coordination.
- Maintain productivity standards consistent with organizational benchmarks.
- Ensure compliance with HIPAA and all state/federal healthcare regulations.
Required Qualifications
- Doctor of Physical Therapy (DPT) degree from a CAPTE-accredited program.
- Active and unrestricted Connecticut Physical Therapist License (issued by the Connecticut Department of Public Health).
- Current CPR/BLS certification.
- Strong knowledge of evidence-based rehabilitation techniques.
- Proficiency in electronic medical record (EMR) systems.
Preferred Qualifications
- 1–3 years of clinical experience (outpatient, inpatient, SNF, or home health depending on setting).
- Board certification (e.g., OCS, NCS, GCS, SCS) through the American Board of Physical Therapy Specialties.
- Experience with value-based care models.
- Bilingual skills (Spanish preferred in many Connecticut markets).
